After you schedule, you will receive a link by email to create an account in our system to complete our forms online. Many patients find that this is more convenient and feels less rushed than completing forms in out office. This also allows the doctor to review your case before you come in which helps us prepare for your visit. If you prefer paper forms, we can provide that in office or mail to you.
